What is the Cosmelan treatment?

Cosmelan is a profes­sional medical depig­men­tation system developed by Mesoestetic and considered the gold standard worldwide for the treatment of hyper­pig­men­tation. It is a two-phase treatment concept that combines intensive outpa­tient treatment at the clinic with a struc­tured home care protocol lasting several weeks.

How it works: The Cosmelan system is based on a multi-layered approach to combating pigmen­tation disorders. At its core is the inhibition of tyrosinase, the key enzyme in the melanin formation process. Blocking this enzyme stops the formation of new pigment at its root. At the same time, Cosmelan stimu­lates the controlled exfoli­ation of the uppermost, pigmented layers of skin, removing any excess melanin that is already present.

The combi­nation of active ingre­dients: Cosmelan contains a scien­ti­fi­cally balanced combi­nation of highly effective depig­men­tation agents:

  • Azelaic acid: Inhibits tyrosinase activity and also has antibac­terial and anti-inflammatory properties. Parti­cu­larly effective for hormone-related pigmen­tation disorders.
  • Phytic acid: A natural active ingre­dient with antioxidant properties that reduces melanin production and also acts as a gentle exfoliant.

The 2-phase concept:

Phase 1 – Outpa­tient treatment: The intensive Cosmelan mask is applied in our practice and remains on the skin for 5-10 hours (deter­mined indivi­dually). This first phase starts the intensive depig­men­tation process.

Phase 2 – Home care: After removing the mask at home, a struc­tured care protocol is followed over several weeks with special Cosmelan home care products. This phase is essential for long-lasting results and prevents the formation of new pigment.

1. Consul­tation and skin analysis

First, we analyse your pigmen­tation disorder in detail: type, intensity, cause and severity. We check whether Cosmelan is suitable for your specific skin problem and what results can reali­sti­cally be expected.

2. Prepa­ration and skin cleansing

On the day of treatment, your skin is first thoroughly cleansed to completely remove make-up, dirt and sebum. A special degre­asing solution is then applied to prepare the skin optimally for the absorption of the Cosmelan active ingredients.

3. Appli­cation of the Cosmelan mask

The intensive Cosmelan mask is now applied evenly and in a suffi­cient layer thickness to the areas to be treated. The mask has a charac­te­ristic dark colour and is carefully distri­buted to ensure optimal delivery of the active ingredients.

4. Struc­tured home care protocol

You will receive a complete care set from us with various Cosmelan home care products and detailed instruc­tions for the next few weeks. The protocol must be followed precisely and includes special cleansing, care and, above all, consi­stent sun protection.

5. Peeling process and regeneration

A controlled peeling process begins within a few days of the treatment. The uppermost, pigmented layers of skin gradually peel off. This process is normal and desirable. No, the Cosmelan treatment is painless. Applying the mask is completely comfor­table. During the appli­cation time, you may feel a slight tightness or tingling sensation, but this is not painful. After removing the mask, the skin may be slightly reddened and sensitive.

The exposure time is deter­mined indivi­dually and is usually between 5 and 10 hours. The exact duration depends on your skin type, the intensity of the pigmen­tation and your skin sensi­tivity. We will determine the optimal exposure time for you.

The first signs of lightening are usually visible after 2–4 weeks. The maximum depig­men­tation effect is visible after 8–12 weeks. The process is gradual – the pigment spots become conti­nuously lighter and fade increasingly.

Yes, a controlled peeling process begins within a few days of treatment. The top layers of skin peel away, revealing lighter, more even skin under­neath. This process is desirable and essential for the result. Important: Do not pull or rub the skin mechanically.

Downtime is minimal. You can generally socialise and go about your daily activities. However, during the peeling phase (appro­xi­m­ately 5–7 days), the skin may be red and peel, which can be cosme­ti­cally disturbing. Ideally, plan the treatment before a quieter period.

Sun protection is absolutely essential – use SPF 50+ every day, even in cloudy weather. Avoid direct sunlight, solariums and saunas for at least 4 weeks. Strictly adhere to the home care protocol. Do not use any additional creams or cosmetics without consulting your doctor first. Medicines containing vitamin A should be avoided.

Sun protection is essential, as UV radiation stimu­lates melanin production and can cause new pigment spots to form. Inade­quate sun protection can severely impair or even negate the results of treatment. Without consi­stent sun protection, pigment disorders can return or worsen.

The struc­tured home care protocol lasts several weeks, usually 8–12 weeks. You will receive precise instruc­tions on which products to use, when and how often. This phase is essential for the success of the treatment and must not be neglected.

Yes, Cosmelan is specially developed for the treatment of melasma and is considered one of the most effective methods against this stubborn form of hyper­pig­men­tation. Hormone-related melasma in parti­cular responds very well to the treatment. Many patients experience signi­ficant lightening or even complete elimination.

Yes, definitely. Cosmelan is parti­cu­larly effective for hormone-related pigmen­tation disorders, such as those caused by pregnancy (chloasma), the contraceptive pill or hormonal changes. The targeted inhibition of tyrosinase inter­rupts excessive melanin production at its root.

Redness and slight swelling are normal in the first few days after treatment and will subside within a few days. The peeling process is desirable and part of the treatment. In rare cases, more severe skin irritation may occur. With correct appli­cation and aftercare, serious side effects are very rare.

With consi­stent sun protection and proper care, the results are very long-lasting. Many patients experience permanent impro­ve­ments. However, new pigment spots may develop with renewed intense sun exposure, hormonal changes or without sun protection. Regular sun protection is therefore important throughout life.

You should refrain from wearing make-up for the first few days after the mask is removed. Once the acute peeling process subsides, you can start wearing make-up again – ideally mineral make-up with sun protection factor. Your home care protocol contains detailed instruc­tions on this.

The treatment should not be carried out during pregnancy or breast­feeding, in cases of active skin diseases, when taking medication containing vitamin A (Roaccutan) or photo­sen­si­tising substances, or if intensive sun exposure is planned in the next 4 weeks (e.g. beach holiday).

In principle, yes, but it is not ideal. We recommend the Cosmelan treatment during the autumn and winter months, as intensive sun protection is essential and this is more difficult to maintain in summer. If you undergo the treatment in summer, you must strictly avoid exposure to the sun.
